Description
The Nurse Practitioner with TridentUSA Mobile Clinical Services will provide for both the acute and chronic health problems of the geriatric and/or long term care resident within the long term care facility or an assisted living facility. The Nurse Practitioner (NP) will be under the supervision of the Regional Director of Clinical Operations and the Medical Director and/or the Primary Care Physician for the resident and will adhere to all state approved medical and nursing guidelines.

Job Responsibilities:
Performs assessment of the resident.
1. Elicits, records and interprets the past medical history, family and social history as it applies to the resident.
2. Performs and records the initial assessment of the resident for the purpose of data entry into the EMR, differentiating between normal and abnormal findings.
3. Continually performs assessment of resident with each follow- up visit as needed.
4. Orders and performs necessary diagnostic tests, procedures and therapeutic interventions as identified in the plan of care.
5. These procedures are to be according to established protocols and standards of care in which the NP is trained and approved to implement by each state Board of Nursing and supervising bodies for NPs.
6. Collects, interprets and records pertinent laboratory and radiographic data.

Plans, implements, and evaluates the management of the resident.
1. Develops a problem list, impression and plan of care for each resident based upon the data obtained from the history, exam and tests / procedures performed.
2. Diagnose and manage acute and chronic health problems.
3. Implements care systems and interventions that reduce patient need for hospitalization.
4. Reviews nursing notes, physician progress notes, notes of other consultative providers and other data pertinent to the care of the resident.
5. Revises the impressions and plan of care based upon subsequent physical exams, laboratory, radiographic findings and overall medical course.
6. Records patient data in the medical record or approved database reflecting any changes in the resident’s condition. The data should include, but not be limited to, physical exam, laboratory and radiographic data, diagnostic impressions and evaluation of implemented plan of care.
7. Communicates information regarding the resident’s condition to nursing staff, attending physician, patient, family and or responsible party. Information can include diagnosis, plan of care and prognosis.
8. Collaborates with other health care team members regarding the provision of care to the resident.
9. Recognizes medical problems and complications that require further physician evaluation and treatment, and refers appropriately to the attending physician and / or specialist.

Assists Director of Nursing (DON) or appointed nursing home liaison with education and program training to nursing home staff.
1. Assists in identifying educational and clinical training needs of department staff.
2. Assists in developing educational and training programs to meet identified departmental needs.
3. Demonstrates and identifies his / her position as an advanced practice nurse in geriatric / long term care to other members of the healthcare team and public.
4. Provide leadership, consultation and education to nursing personnel and other health care providers regarding preventive health care, routine follow up, and urgent/acute care.
5. Is available to participate in meetings with clinic staff that are medically appropriate, with patient specific needs, and a goal to improve patient outcomes and staff communication.

Performs and assumes responsibility for administrative functions pertinent to the NP’s individual practice.
1. Obtains correct demographic and insurance information in a timely manner, abiding by company policies and protocols.
2. Is responsible for learning/knowing all billing/coding protocols/procedures pertinent to his or her own practice.
3. Learns and performs functions of TridentUSA Mobile Clinical Services software on a timely basis.
4. Utilizes TridentUSA/Mobilex/ADP services for human resource related needs including time off and timesheet approval.
5. Meets with collaborating physician in timely manner according to state rules / regulations and completes all documentation in regards to this in timely manner.
6. Is able to effectively utilize a multi-function machine (scan, copy, and fax), laptop computer and other devices as needed for proper and timely implementation of practice.
7. Is able to have a home based office with storage of supplies for the primary care practice, as well as necessary supplies for continence and foot care services as needed.
8. Works in partnership with the Practice Manager (PM) who is responsible for a majority of the above named functions. PMs are not allowed to have patient contact.
9. Follows all TridentUSA policies and operations protocols.
10. Responsible for company credit cards assigned and completes expense reports on a timely basis.
11. Returns all Credentialing and other paperwork that is required of the Nurse Practitioner by the TridentUSA/Mobilex corporate staff that day or within the timeframe requested

Assumes responsibility for own professional growth through continuing education.
1. TridentUSA will pay expenses for one continuing education conference relating to primary care as approved by the Regional Director of Operations.
2. Other continuing education seminars are to be approved through the NP’s immediate supervisor.
3. The NP is to be responsible to obtain all necessary CEU’s in order to keep licensure and certification current according to his or her state regulations and the regulations of the appropriate credentialing body as well as providing electronic copies for administrative credentialing staff at TridentUSA. Copies of all completed CEUs should be sent to the attention of Alaina Byrne at the TridentUSA Mobile Clinical Services Corporate office.
4. State certification and credentialing must be maintained.
5. The NP is responsible for ensuring that licenses and credentials are up to date.

Participation in Healthcare Effectiveness / QI Programs:
1. All NPs must be aware of their respective state rules and regulations which govern their practice.
2. Each NP is to ensure that he or she is in compliance with all state requirements regarding the oversight/supervision of his or her practice.

Qualifications / Job Requirements:
1. Graduate degree in nursing. Current registered nursing license in state position is located. Appropriate registrations/approvals to practice as APN in appropriate state. Prior NP experience is desired, but not required.
2. Good communication skills, patience, and the willingness to handle situations with empathy and professionalism.
3. Basic computer skills are a necessity.
4. Valid driver license is required.
5. Reliable transportation and car insurance (enforce) is necessary and a job requirement.
6. Ability to travel up to two hours (sometime more) from home for work or training assignments and stay overnight if needed for training or covering for another NP.
7. Reliable transportation and car insurance (enforce) is necessary and a job requirement.
8. Ability to travel up to one hour and half (sometime more) from home for work or training assignments if necessary.
9. Able to work in other territories, when the need arises, within the states that they are credentialed, to assist in covering markets temporarily outside of their own territory to cover for another NP due to illness, vacation, position vacancy or other company need when identified by TridentUSA staff.
10. Occasional out-of-state travel is required for company meetings, continuing education opportunities and / or training.
11. Must be punctual for the first nursing home appointment of the morning, and any nursing home appointment thereafter.
12. Must utilize TridentUSA nursing home software / PCEMR as instructed without modification or variation in practice.
13. Read and respond to all emails and requests daily and in a timely manner.
14. Be available by phone or Skype during work hours.
15. Be available by phone until 8:00pm each workday for any questions on any orders given that day.
16. Other duties as assigned.

• Standing / bending / lifting up to 40 lbs.
• Long / flexible hours
• Ability to drive long distances
